ACTIVITY SWEATSHIRT

The activity sweatshirt is a gray hooded sweatshirt with the logo seen above on the front.  The cost of $30.00 includes a free white t-shirt with a matching logo.  There are six sizes available: small, medium, large, x-large, xx-large, and xxx-large.  There is an additional $4.00 charge on XXL and XXXL sweatshirts. Students will have the option of putting their LAST NAME ONLY on the back of the sweatshirt for an additional $6.00.  Students have the option to include graphic decals that represent the activities that they participate in at Columbia.  Decals are $2.00 each and will be placed on the sleeves of the sweatshirt.  Students will be unable to add decals in the future so if you are planning to play a winter sport, go ahead and include the graphic for that sport.

Order forms need to be filled out completely and correctly. Payment is due at the time the order is placed.  Order must be turned into Mrs. Wood during lunch.
