Athletic Participation Forms

All FHS athletic participation forms can be accessed and printed below.  All forms must be turned into the office prior to participation.

• Parent/Student Activity Participation Statement  
• FHS Athletic/Activity Code  
• Physical Exam Form {Physicals must be renewed every 24 months and must be turned into the office before students may participate in practices or games.}
• Parent/Student Activity Participation & Medical Authorization Form {As family information can (and does) change from one season to the next, this form must be filled out completely and signed by the student and parent prior to each sport season in which your child wishes to participate.}
• E-Card
